About the Role 
Not just any Case Manager position.  For this role we are looking for a passionate Case Manager with strong skill-set in program growth through community engagement & creative approaches within the community.
We want you to use your strong personal skills to push our business to the next level. These kind of opportunities don’t come along every day and the career opportunities for this role are endless as we evolve this program to the next level!  You will be part of a collaborative and friendly team, pushing towards the same goals. This is a mobile role,  with a sales aspect as you market the services of our Aged Care programs to partners in the industry. Based out of the Dandenong LWB offices with travel primarily in the South East with some ad hoc requirements in other areas. 
  
You will be responsible for building program growth through community engagement, sales & growth opportunities you identify and a developing and implementing care plans in partnership with the consumer/representative across all of our Aged Care programs across the state.  While drawing from your tertiary qualifications and experience, you will have a demonstrated understanding of current Consumer Directed Care principles, experience with working in the community Aged Care sector, as well as having strong communication and mediation skills and a current driver's licence. 

Key Responsibilities
  • Drive program growth through engagement projects you will oversee
  • Ensure appropriate information is given regarding the home care agreement and rights and responsibilities in acceptance of a home care package.
  • Represent Life Without Barriers and the Aged Care Program at external forums and meetings and promote the program within the community
  • Participate in regular individual supervision, performance reviews and team reflective practice sessions.
  • Support and promote the work of Life Without Barriers, maintaining a positive image of the organisation in accordance with level of position.
  • Monitor consumer budgets in relation to program guidelines.
  • Undertake comprehensive consumer assessment and care planning within a psychosocial model.
  • Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
  
Skills & Experience
  • Demonstrated ability to engage in community engagement, sales & growth activities
  • Degree or equivalent in allied health, nursing, or a related field
  • Demonstrated experience of working with frail elderly, people with culturally and linguistically diverse backgrounds and understanding of issues facing people with multiple and complex issues
  • Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for frail elderly, people with dementia and their carers, people with culturally and linguistically diverse backgrounds
  • Highly developed analytical skills and the ability to apply these skills in case management
  • Demonstrate alignment to LWB values
  • High level communication, influencing, liaison, negotiation and mediation skills
  • Current Driver's Licence
  
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Victorian Working With Children check.
